Passport Application Services
Passport Services are available at all Community Service Centers. Application processing hours vary at each site.
Application Requirements
- All applicants must apply in person. Applications are available at the Service Center
or can be downloaded off the State Department's web site: http://travel.state.gov/passport/passport_1738.html
Complete the application through section #22. DO NOT SIGN APPLICATION.
- Proof of Citizenship
- Previous U.S. Passport
- Certified ORIGINAL Birth Certificate, if citizen born in United States OR
- ORIGINAL Naturalization Certificate, if naturalized citizen OR
- Consular Report of Birth Abroad
- Proof of Identity
- Current Driver's License OR
- United States Passport OR
- Certificate of Naturalization OR
- Official Military, State Issued or Government Employee I.D. Card
- Two Passport Photos

U.S. Passport Book (Valid for travel by air, sea, or land).
Adult (16 years & older)
Passport Fee/Security Fees $75.00 (made payable to U.S. Department of State)
Execution Fee $25.00 (made payable to City Treasurer)
Child (15 years & younger) - Two parent consent is now required for minors under the age of 16.

Execution Fee $25.00 (made payable to City Treasurer)
U.S. Passport Card (Valid for land and sea crossings between the U.S. and Canada, Mexico, Bermuda, and the Caribbean only. NOT VALID FOR TRAVEL BY AIR).

Execution Fee $25.00 (made payable to City Treasurer)

Processing Time
Typically, passports are processed and mailed to applicants within 4-6 weeks.
For an additional $60 fee the application can be expedited and the passport processed in 3-4 weeks.
